ECG Strips: Rhythm Blitz

Rapid 60-second telemetry recognition sprint. Interpret authentic dynamic waveforms, measure P-QRS-T intervals with digital calipers, and master instant NCLEX/ACLS emergency interventions.

14 Clinical Rhythms

From Sinus Tachy to Lethal V-Fib, Asystole, A-Flutter, and 1st/2nd/3rd degree AV blocks.

Two-Tier Scoring

Diagnose the rhythm for quick points, then nail the critical first-line NCLEX nursing priority!

60s Blitz & Streak Multipliers

+3s bonus for speed. Build up to 3x combo multipliers. -4s penalty for dangerous misidentifications.

High-Yield Reference Guide

The 6-Step Systematic ECG Interpretation Method

When analyzing any Lead II rhythm strip on NextGen NCLEX, UK CBT, or bedside ICU telemetry, follow this standardized 6-step clinical algorithm to avoid misdiagnosing life-threatening dysrhythmias.

1Heart Rate (BPM)

Regular Rhythm: Divide 300 by number of large boxes between R waves (e.g. 4 large boxes = 75 bpm).
Irregular Rhythm: Count the number of R waves in a 6-second strip (30 large boxes) and multiply by 10.

2Rhythm Regularity

Use paper or calipers to measure R-R intervals across the strip. Are they constant (Regular) or variable?
Irregularly irregular = Atrial Fibrillation. Patterned grouped beating = Wenckebach or Bigeminy.

3P Waves & Atrial Depolarization

Are P waves present? Are they upright in Lead II? Is there exactly one P wave before every QRS complex (1:1 ratio)?
Sawtooth flutter waves = Atrial Flutter. Fine chaotic baseline = A-Fib. Independent P waves = 3rd Degree Block.

4PR Interval (Normal: 0.12 - 0.20s)

Measure from beginning of P wave to beginning of QRS. Normal is 3 to 5 small boxes (0.12 - 0.20s).
PR > 0.20s = First-Degree AV Block. Progressive PR prolongation = Wenckebach (Mobitz I).

5QRS Duration (Normal: < 0.12s)

Normal narrow QRS is < 3 small boxes (< 0.12s), indicating supraventricular conduction through the bundle branches.
Wide QRS (≥ 0.12s) indicates ventricular rhythm (V-Tach, PVCs, or Bundle Branch Block).

6ST Segment & T Wave Evaluation

ST Elevation (≥ 1mm) indicates acute STEMI myocardial infarction (tombstoning).
ST Depression / T inversion indicates myocardial ischemia. Tall peaked symmetric T waves indicate hyperkalemia!

The Essential NCLEX AV Heart Block Rhymes

Memorize these 4 clinical rhymes to instantly differentiate all AV nodal conduction blocks on board exams:

1st Degree AV Block
“If R is far from P, then you have a First Degree!”
PR interval prolonged >0.20s, constant on every beat, 1:1 ratio.
2nd Degree Mobitz I (Wenckebach)
“Longer, longer, longer, drop! Then you have a Wenckebach!”
Progressive PR prolongation cycle by cycle until a QRS drops.
2nd Degree Mobitz II
“If some Ps don’t get through, then you have Mobitz Two!”
Constant PR interval on conducted beats; sudden unexpected dropped QRS. High risk of 3rd degree!
3rd Degree Complete Block
“If Ps and Qs don’t agree, then you have Third Degree!”
Complete AV dissociation. P waves and QRS march independently. Emergency pacemaker required!
