Clinical Practice Pearls & Safety Highlights

  • Aggregates 6 physiological parameters: Respiration rate, Oxygen saturation (Scale 1 or 2), Systolic BP, Pulse rate, Consciousness (ACVPU), and Temperature.
  • Scale 2 Oxygen Saturation targets (88โ€“92%) are reserved exclusively for patients with confirmed hypercapnic respiratory drive (e.g. severe COPD).
  • Aggregate score of 5 or more, or a RED score (3 in any single parameter), triggers an urgent clinical response by a doctor or critical care outreach team.

Physiological Parameters & NEWS2 Scoring Matrix

Each parameter is scored from 0 to 3 based on deviation from normal physiological ranges. Scores reflect physiological stress and risk of cardiac arrest or unplanned ICU admission.

Clinical Risk Tiers & Escalation Response

Ward nursing staff must execute escalation protocols according to the aggregate score: โ€ข Low Score (0): Minimum 12-hourly monitoring. โ€ข Low Score (1โ€“4): Minimum 4โ€“6 hourly monitoring; inform registered nurse to assess need to increase frequency. โ€ข Low-Medium (RED 3 in a single parameter): Urgent review by ward nurse and attending junior doctor within 1 hour. โ€ข Medium Score (5โ€“6): Urgent review by doctor with core competencies in acute care; minimum 1-hourly monitoring. โ€ข High Score (7+): Immediate emergency assessment by Critical Care Outreach Team (CCOT) or Medical Emergency Team (MET); continuous monitoring of vital signs and transfer to Level 2/3 care if indicated.

The SBAR Escalation Communication Framework

When calling a doctor or outreach team for a deteriorating patient, standardise handover using SBAR (Situation, Background, Assessment, Recommendation) to prevent critical delays.
